Forms and DocumentsAll required forms must be completed and submitted no later than three (3) days prior to the first scheduled training session or petsitting visit. This allows our staff adequate time to review your pet's history and needs.Failure to submit all required documentation at least three (3) days before the scheduled visit may result in cancellation or postponed service. Any rescheduling due to incomplete paperwork will be treated as a late cancellation and will be subject to the applicable $50 late cancellation fee and the forfeit of payments or deposits previously made.
